These downloadable forms are the typical documents which are required to enroll your child at Penn Pals.  Please be aware that you must check availability, speak with one of the directors and preferably visit the center before you enroll your child.
These downloads are provided for your convenience.  Many of them are forms that you will need to update on a regular basis.

Emergency Contact Form

This form is used to provide us with all of the emergency contact information, as well as insurance and special needs for your child.

 

 

Child Care Agreement

This form is completed by the site Director and it will detail your child care fees.  It will be updated any time your care changes or every six months.

 

Registration Form

This form, along with your registration fee will hold your spot for enrollment.

 

 

Child Health Report

This form is to be filled out by your child's doctor.  It is due within 30 days of enrollment and is to be updated each time your child has a well-visit, typically at 3m,6m,9m,12m,18m,2y,3y,4y,5y,etc.

 

Parent Handbook

This handbook will tell you all you need to know to have a rewarding experience at Penn Pals.

 

Getting To Know You

This form will be filled out upon enrollment and will help us get to know you and your child.
